All patients should understand that any patient seeking medical treatment is not considered a patient of the practice until our Provider has completed a review of the patient’s Medical History and an In-Person assessment of that patient in our office. 

As we see patients for Urgent Care and Primary Care services, it is important to note that patients are not considered Primary Care patients of our practice unless they have completed the Primary Care Intake History & Physical by one of our Providers and have then been notified by the practice that they are now our Primary Care Patient. 

In the case of Urgent Care patients, it is strongly advised that patients follow up with their individual primary care provider on a regular basis for all other medical issues that are not related to the episodic medical visit they had with Carolina Urgent & Family Care.  Urgent Care patients are also advised to follow up with the primary care provider for any continuing care associated with the visit they had at Carolina Urgent & Family Care.
